AllianceHealth Clinton Announces Custodian for Patient Records

As AllianceHealth Clinton plans to end all hospital and affiliated clinic operations on December 31, 2022, the organization is making arrangements for patient medical records to be retained in accordance with state of Oklahoma guidelines. Once the AllianceHealth operations have ended, medical records will be transferred to AllianceHealth Durant which will act as custodian.

After December 31, patients can request medical records for care received at AllianceHealth Clinton by contacting:

You may also request medical records online. Visit AllianceHealthDurant.com, select Patient & Visitors, then click on Request Medical Records. Patient portal access will continue to be available after December 31, 2022 using the same MyHealthHome account you have already established.

All requests for medical records must be in writing. Copy fees may apply.

Mammography

If you had your mammograms performed at AllianceHealth Clinton and need those images transferred to another facility, please call the AllianceHealth Durant Medical Records Department at 580-931-2045 to complete a records release to have the images sent to the new location. You can also provide the Durant Medical Records phone number to the facility where you will be going for your next mammogram and they can call Durant directly to get the images released.
